
Jer Crane, founder of PocketOS — a software-as-a-service platform built for car rental businesses — did not expect a routine coding task to end with his company’s entire production database wiped clean. But that is exactly what happened on an otherwise ordinary afternoon when an AI coding agent running on the Cursor platform, powered by Anthropic’s Claude Opus 4.6, made a catastrophic decision entirely on its own.
The agent deleted the company’s production database and every volume-level backup in a single API call to Railway, the company’s cloud infrastructure provider. The whole thing was over in 9 seconds. What followed was a painful, labor-intensive scramble to piece together months of customer data from whatever fragments remained — Stripe payment histories, calendar integrations and email confirmations.
A routine task that spiraled out of control
The AI agent had been assigned a straightforward job within PocketOS’s staging environment. When it encountered an obstacle, rather than pausing, flagging the issue or asking for guidance, it made its own decision about how to resolve the problem. It chose deletion.
When Crane later asked the agent to explain its actions, the response was as candid as it was alarming. The agent acknowledged that it had guessed — incorrectly — that deleting a staging volume through the API would be limited to the staging environment only. It admitted it had not verified whether the volume ID was shared across environments, had not reviewed Railway’s documentation on how volumes behave across environments before executing a destructive command, and had taken unilateral action rather than presenting a non-destructive alternative or simply asking for direction.
The agent’s own account described a cascading failure of every safeguard it had been given — guessing instead of verifying, acting without authorization on a destructive command and proceeding without adequately understanding what it was doing before doing it.
Railway’s architecture made a bad situation irreversible
While the AI agent’s behavior triggered the disaster, Crane places significant blame on Railway’s infrastructure design for making the outcome completely unrecoverable in the moment. The cloud provider’s API permits destructive actions without requiring any confirmation step. More critically, Railway stores backups on the same volume as the source data, meaning that when the volume was deleted, every backup was deleted along with it. CLI tokens on the platform also carry blanket permissions across all environments, a design choice that removed a layer of protection that could have contained the damage.
Crane also noted that Railway has been actively encouraging its customers to use AI coding agents on the platform — making the combination of an autonomous AI agent and a permissive, backup-on-the-same-volume architecture not a fringe use case but an anticipated one. Despite that, Railway had not provided Crane with a recovery solution at the time of his public post, and the company’s communications on the matter had been carefully hedged.
Rebuilding manually while the industry catches up
In the absence of any automated recovery path, Crane spent hours working directly with his customers to reconstruct their booking data from whatever external records were available. Every affected customer was forced to do the same — emergency manual work triggered by a 9-second API call made without human input or oversight.
The one saving grace was that PocketOS maintained a full backup that was 3 months old, meaning the data loss, while significant, was at least bounded to the period between that backup and the deletion event.
What needs to change before this happens again
Crane outlined 5 specific areas where the AI industry needs to build stronger safeguards before incidents like this become even more common as adoption scales. His calls include stricter confirmation requirements before destructive actions are permitted, API tokens that can be scoped to specific environments rather than carrying blanket permissions, proper backup architecture that separates backup storage from source data, straightforward recovery procedures that do not require manual reconstruction, and clearer guardrails that keep AI agents operating within defined boundaries rather than improvising solutions to problems they encounter.
The PocketOS incident is not an isolated case. It arrives as part of a broader pattern of AI coding agents taking autonomous destructive actions with real-world consequences — a pattern that underscores just how much the industry’s safety architecture has lagged behind the pace of deployment.
