
The director of HBO’s The Pitt opens up about his creative journey, his Emmy win, and why telling the truth on screen has always been his greatest priority
Long before Damian Marcano was directing some of television’s most talked-about episodes, he was a kid in Port of Spain, Trinidad, telling stories with friends under a mango tree. There were no cameras, no crews and no scripts just imagination and a deep instinct for what made a moment feel alive. That instinct, it turns out, never left him.
Today, Marcano is an Emmy Award-winning director whose work on HBO’s critically praised medical drama “The Pitt” has earned him recognition far beyond what he ever expected when he first arrived in the United States. His episodes have drawn attention for their immersive, ground-level realism a quality he says is entirely intentional and deeply personal.
Finding his way to ‘The Pitt’
Marcano’s path to the show was not a straight line. His working relationship with producer John Wells and his team began with The Emperor of Ocean Park, a limited series filmed in Chicago with Forrest Whitaker. It was during that production that an executive producer reached out with a new set of scripts, asking if he would consider a conversation about a different project Wells had in mind. That project became “The Pitt.”
Medical drama was not something Marcano had been chasing. His background and creative instincts had always pulled him toward stories rooted in the kind of vivid, high-stakes energy he associated with where he came from. But something about the show’s approach changed his mind, and he credits that shift entirely to the material itself. He went from being someone with little interest in the genre to someone who has become, by his own admission, a genuine fan of it.
What drew him in, he says, was the show’s open door spirit the idea that the emergency room, like the communities he grew up around, welcomes everyone without exception. That parallel between the ER and his Rastafarian values gave him a framework he could connect to authentically.
The craft of staying honest
For Marcano, directing is fundamentally about truth. Before cameras roll on any scene, he gathers the actors involved and simply has them read through the material. He wants to hear it before he shapes it. His goal, particularly on a show like “The Pitt,” is to create something so grounded that audiences feel they are physically present in the room not watching from a comfortable distance.
He describes his ideal episode as something that could be revisited a decade from now and still feel like an accurate document of what an emergency room day actually looked like. That kind of lasting honesty, he says, is what he is always reaching for.
Working within an episodic format also requires a specific kind of discipline. With multiple directors rotating across a season, maintaining tonal consistency is essential. Marcano is deliberate about understanding where his episode fits in the larger arc, ensuring that character behavior feels continuous rather than reinterpreted. He compares it to a relay race taking the baton cleanly and passing it just as carefully.
An Emmy he never saw coming
When the Emmy came, Marcano was still deep in production on another project, directing episodes of Netflix’s “All the Sinners Bleed” for a production team that includes the Obamas and Steven Spielberg. He describes the award as something that genuinely caught him off guard not because he lacked confidence in his work, but because growing up in Trinidad, the Emmy simply was not part of his frame of reference. He knew what an Oscar was. The Emmy felt like a different world entirely.
What made the moment matter most to him was not the trophy itself, but what it became for his sons. His wife sent him a photo while he was on the road, and he realized that his boys had started stopping by the family library each morning to look at the shelf where the Emmy sits alongside a Golden Globe, a Critics Choice Award and a PGA Award. For someone who describes himself as skeptical of competition in the arts, that image his children finding inspiration in what he had built turned the win into something genuinely meaningful.
What drives him forward
Marcano’s episode 13 of “The Pitt,” which features a script written by an actual emergency room physician, reflects exactly the kind of immersive storytelling that has defined his work on the series. Access to real medical voices allows him to ask the questions that matter most how healthcare workers sustain themselves through grueling shifts, what keeps them returning and what it costs them emotionally to do so.
Those questions, he says, are the ones worth putting on screen. And the closer he gets to an honest answer, the better the work becomes.
